癌症患者初次诊断心理痛苦现状及影响因素分析

目的探讨癌症患者初次诊断心理痛苦现状及影响因素,为其诊疗阶段心理干预提供参考。方法选取2017年3月至2019年2月我院初次诊断为癌症的102例患者为研究对象,以问卷调查的方式,总结分析患者的一般资料、心理痛苦管理筛查工具(DMSM)评分,了解其心理痛苦现状,确定其独立影响因素。结果初次诊断为癌症患者的心理痛苦温度计(DT)平均分为(4.25±0.49)分,DT≥4分占59.80%;在心理痛苦相关因素列表(PL)中,占比最多的因素分别为经济因素、疲乏、担忧,占比分别为51.96%,38.24%,65.69%;多元线性回归分析结果显示,性别、年龄、婚姻状态、家庭居住地、月收入、疼痛情况对DT评分有影响(P<0.05)。结论癌症初次诊断在得知患病后,会产生强烈心理应激,往往承受着巨大的心理痛苦,探讨引起癌症初次诊断心理痛苦的现状,分析影响因素并予以个体化心理干预,对康复及预后改善具有重要意义。

Analysis the current status and influencing factors of psychological pain in the first diagnosis of cancer patients

Objective To explore the current status and influencing factors of psychological pain in the first diagnosis of cancer patients,and to provide reference for psychological intervention in the diagnosis and treatment stage.Methods We selected 102 cases of patients who were diagnosed with cancer for the first time in our hospital from March 2017 to February 2019 as the study objects.The questionnaire survey was used to summarize and analyze the general information and distress management screening measure(DMSM)scores of the patients to understand their status of psychological distress and determine their independent factors.Results The distress thermometer(DT)score of patients with cancer diagnosed for the first time was averaged(4.25±0.49)points,and DT≥4 points accounted for 59.80%.In the problem list(PL)of psychological pain related factors,the most common factors were economic factors,fatigue and worry account for 51.96%,38.24%,and 65.69%respectively.Multiple linear regression analysis showed that the gender,age,marriage status,family residence,monthly income,and pain had an impact on the DT score(P<0.05).Conclusion The first diagnosis of cancer would produce strong psychological stress after learning of the disease,and often suffers from huge psychological pain.Explore the current status of the psychological pain caused by the first diagnosis of cancer,and analyze the influencing factors and personalize psychological intervention,which was of great significance for improvement of prognosis.
