急诊重症监护病房患者发生ICU综合征的影响因素分析与防护对策

目的调查分析急诊重症监护病房(EICU)患者发生ICU综合征的影响因素与防护对策。方法选取2017年2月至2019年2月我院收治的EICU患者183例,其中41例发生ICU综合征。采用单因素和多因素logistic回归分析影响EICU患者发生ICU综合征的危险因素。结果APACHE II评分、高血压或脑梗死史、ICU环境压力源量表评分、使用机械通气是EICU患者发生ICU综合征的独立危险因素(P<0.05),家属或社会支持、术前综合宣教是EICU患者发生ICU综合征的保护因素(P<0.05)。结论EICU患者发生ICU综合征与人口学资料、治疗情况及环境状况等诸多因素相关,医护人员需重视病情评估,联合多学科全面诊断治疗,密切关注物理、人文、治疗环境对患者心理状况产生的效果,从而避免ICU综合征发生,降低病死率。

Analysis of influencing factors of ICU syndrome in patients with emergency intensive care unit and its protective measures

Objective To investigate the influencing factors of ICU syndrome in patients with emergency intensive care unit(EICU)and its protective measures.Methods To select 183 cases of patients with EICU admitted to our hospital from February 2017 to February 2019,of which 41 cases developed ICU syndrome.The univariate and multivariate logistic regression analysis was used to analyze the risk factors of ICU syndrome in patients with EICU.Results The APACHE II score,hypertension or history of cerebral infarction,ICU environmental stressor scale score,and mechanical ventilation were the independent risk factors for ICU syndrome in patients with EICU(P<0.05).Family members or social support and preoperative comprehensive education were protective factors for ICU syndrome in patients with EICU(P<0.05).Conclusion The occurrence of ICU syndrome in patients with EICU was related to many factors such as demographics,treatment status,and environmental conditions.Medical staff need to attach importance to the assessment of the condition,combine comprehensive multi-disciplinary diagnosis and treatment,and pay close attention to the physical,humanity,and therapeutic environment that affected the effect of patient's psychological condition,thereby avoiding the occurrence of ICU syndrome and reducing mortality.
