多维度镇痛对分娩过程及结局的影响

目的探讨多维度镇痛对分娩过程及结局的影响。方法选取2017年7月至2018年8月在我院分娩的110例产妇作为研究对象,随机等分为试验组和对照组,试验组在分娩全程中采用多维度镇痛,对照组产妇宫口开张≥3 cm,进入活跃期之后进行活跃期硬膜外阻滞麻醉,观察两组产妇产程镇痛效果以及分娩结局。结果试验组产妇潜伏期VAS评分与对照组相比明显较低(P<0.05);试验组活跃期时间、第二产程时间及缩宫素使用率明显低于对照组(P<0.05);试验组剖宫产率低于对照组(P<0.05);在第二产程时,试验组外周血β-EP高于对照组、去甲肾上腺素(NE)和血糖浓度均明显低于对照组(P<0.05);两组产妇新生儿Apgar评分比较差异无统计学意义(P>0.05)。结论全产程多维度镇痛可有效降低产妇分娩过程中的疼痛程度,降低剖宫产的发生率,是临床上安全有效的镇痛方法,值得应用和推广。

Effects of multi-dimensional analgesia on the delivery process and outcome

Objective To discuss the effect of multi-dimensional analgesia on the delivery process and outcome.Methods To select 110 cases of parturients who were delivered in our hospital from July 2017 to August 2018 as the study objects,and they were randomly divided into an experimental group and a control group.The experimental group adopted multi-dimensional analgesia throughout the delivery process,and when the uterine orifice of pregnant women had a opening of≥3 cm,the control group performed anesthesia for epidural block during active phase after entering the active phase to observe the analgesic effect and delivery outcomes of pregnant women of the two groups during deliver process.Results The incubation VAS score of the parturients of the experimental group was significantly lower than that of the control group(P<0.05).The time of active phase,the second stage of labor duration and the use of oxytocin in the experimental group were significantly lower than the control group(P<0.05).The rate of cesarean section in the experimental group was significantly lower than that in the control group(P<0.05).During the second stage of labor,the peripheral bloodβ-EP in the experimental group was higher than that in the control group,and the norepinephrine(NE)and glucose concentration were significantly lower than that in the control group(P<0.05).There was no significant difference in neonatal Apgar score between the two groups(P>0.05).Conclusion The multi-dimensional analgesia throughout the delivery process can effectively reduce the pain during the delivery process of the parturient,and reduce the incidence rate of cesarean section.It was a clinically safe and effective analgesic method,which was worth popularizing and applying.
