基于快速康复外科理念的护理干预在全髋关节置换术后下肢深静脉血栓预防中的应用

目的探讨基于快速康复外科(FTS)理念的护理干预在全髋关节置换术后预防下肢深静脉血栓(DVT)中的应用效果。方法选取2018年5月至2019年5月70例行全髋关节置换术患者为研究对象,随机将其等分为对照组与研究组,对照组采用常规护理,研究组采用基于FTS理念的护理干预,对比两组汉密尔顿焦虑量表(HAMA)评分、疼痛视觉模拟评分法(VAS)评分、Harris髋关节功能评分(HHS)、活动度和畸形评分。结果术后7 d,研究组HAMA评分、VAS评分显著低于对照组(P<0.05);术后3个月,研究组的HHS疼痛、功能、活动度评分均高于对照组,畸形评分低于对照组(P<0.05);术后7 d,研究组DVT发生率显著低于对照组,差异均有统计学意义(P<0.05)。结论基于FTS理念的护理干预应用于全髋关节置换术术后预防下肢深静脉血栓的发生,效果显著,可改善下肢关节功能,减轻术后疼痛,值得推广。

Application of nursing intervention based on fast track surgery concept in prevention of deep vein thrombosis of lower limb after total hip replacement

Objective To explore the application effect of nursing intervention based on fast track surgery(FTS)concept in prevention of deep vein thrombosis(DVT)of lower limb after total hip replacement.Methods To select 70 cases of patients undergoing total hip replacement from May 2018 to May 2019 as the study objects,and they were randomly divided into a control group and a study group.The control group adopted routine nursing,and the research group adopted nursing intervention based on FTS concept to compare the Hamilton anxiety scale(HAMA)score,visual analogue scale(VAS)score,Harris hip function score(HHS),mobility and deformity score of the two groups.Results Seven days after surgery,the HAMA score,and VAS score of the study group were significantly lower than those of the control group(P<0.05);three months after surgery,the HHS pain,function,and mobility scores of the study group were higher than those of the control group,and the deformity score also was lower than that of the control group(P<0.05);seven days after surgery,the incidence of DVT of the study group was significantly lower than that of the control group,and the differences were statistically significant(P<0.05).Conclusion The application of nursing intervention based on the FTS concept in prevention of DVT of lower limb after total hip replacement,had a significant effect,and can improve the joint function of lower limb and reduce postoperative pain,so it is worth promoting.
