失效模式与效应分析在手术室护理质量安全管理中的应用效果

目的探讨失效模式与效应分析(FMEA)在手术室护理质量安全管理中的应用效果。方法以我院普通外科手术室护理情况以及78名器械护士和巡回护士作为研究对象,2017年1月至2018年1月实施常规护理质量安全管理模式,2018年2月至2019年2月在常规模式基础上实施FMEA,比较FMEA实施前后手术不良事件发生情况、器械护士和巡回护士工作质量情况。结果FMEA实施后体位相关并发症、手术感染、锐器损伤、手术用物清点数目不符、外科手消毒不规范、医护患沟通不足发生率低于实施前(P<0.05);FMEA实施后器械护士和巡回护士工作质量合格率均高于实施前(P<0.05)。结论FMEA在手术室护理质量安全管理中应用效果显著,可有效降低手术风险,减少术后不良事件发生率,提升护理服务质量,值得临床推广应用。

Application effect of failure mode and effect analysis in nursing quality and safety management in operating room

Objective To explore the application effect of failure mode and effect analysis(FMEA)in nursing quality and safety management in operating room.Methods To take the nursing situation,78 instrument nurses and circulating nurses in the general surgery operating room of our hospital as the study objects.The routine nursing quality and safety management model was implemented from January 2017 to January 2018,and the FMEA based on the routine model was performed from February 2018 to February 2019 to compare the incidence of adverse surgical events before and after implementing the FMEA and the work quality of instrument nurses and circulating nurses.Results After implementing FMEA,the incidence of postural-related complications,surgical infections,sharps injuries,inconsistent number of surgical materials,non-standard surgical hand disinfection,and the insufficient communication between medical,nursing staff and patients were lower than those before implementing FMEA(P<0.05).After implementing FMEA,the qualification rate of the work quality of instrument nurses and circulating nurses were higher than those before implementing FMEA(P<0.05).Conclusion The FMEA had a significant effect in the management of nursing quality and safety in the operating room,which could effectively reduce the risk of surgery,reduce the incidence rate of postoperative adverse events,and improve the quality of nursing services.It was worthy of clinical promotion and application.
