电视腹腔镜逆行与顺行胆囊切除术的对比观察

目的比较LC术中逆行切除胆囊与顺行切除胆囊的临床效果。方法将同期分别采用两种方法手术的病例各提取2000例进行统计比较。A组为逆行切除组，B组为顺行切除组。结果平均手术时间：A组为20．56分钟，B组为26．36分钟（P〈0．05）；中转开腹率：A组为7例（0．35％），B组为17例（0．85％，P〈0．05），差异有统计学意义。结论通过大综病例样本统计，LC手术中逆行切除胆囊有其较大的优越性，能够缩短手术时间，有效地减少中转开腹率，特别是能减少和避免胆道损伤这一LC术中常见严重并发症的发生。

Comparison of Laparoscope Retrogradation Cholecystectomy With Conventional Cholecystectomy

Objective To compare the retrogradation cholecystectomy and conventional cholecystectomy by laparoscope （LC）. Method 4000 cases was divided into 2 group, A group（N=2000）patients accepted retrogradation cholecystectomy, B group （N = 2000） patients experienced conventional cholecystectomy. The clinical date of both 2 groups is analyzed by statistical comparison. Results Mean operation time is 20. 56 min and 26. 36 min for A group and B group（P〈0. 05）,respectively. The percentage of patients who must be operated by open peritoneal cavity during the LC operation is 0. 35 % （N=7） vs. 0.85 % （N= 17） （A group vs. B group）（P〈0.05）. Conclusion It is indicated that retrogradation cholecystectomy has more superiority and shorter operation time,lower percentage of patients who must be operated by open peritoneal cavity than conventional cholecystectomy,besides,it can especially reduce and avoid the trauma of billiard tract which is the common severity complication during LC.
