股骨头坏死塌陷预测方法的临床意义

目的 探讨塌陷预测方法在股骨头坏死临床评价中的意义。方法 2006年6月至2009年10月，选取30例37髋未予系统治疗的股骨头坏死患者，用以对比和评估几种主要塌陷预测方法在股骨头坏死塌陷预测中的价值，从而选择最佳预测法预测塌陷风险和评价保髋治疗的临床疗效。结果 坏死分型属于B型，则用正位分型法较准确；如属于中心性坏死，用坏死形态预测法较准确；若正位分型属于C1、C2型，则同时应用改良Kerboul角度法、坏死区体积百分比法、正位分型法进行塌陷风险预测。结论 不同塌陷预测方法各有利弊，不能单独使用，应围绕正位分型法进行综合分析，早期诊断是提高中医药保髋疗效的关键。

The significance of collapse prediction in clinical assesment of the femoral head necrosis

Objective Explore the significance of collapse prediction in clinical assesment of the femoral head necrosis. Method From June 2006 to October 2009, there were 30 patients with 37 hips sufferd from ONFH. All patients (37 hips) were not systematic treatment used to compare and evaluate the value of several major collapse prediction way in predicting collapse; collapse risk rate and preserving hip treatment were used to evaluate clinical efficacy which depended on the best predicting method. Result Orthotopic typing is more accurate if the necrosis type is B; then prediction method of the necrotic shape is more accurate belonging to central necrosis；To predicte the risk of collapse combined with the method of modified Kerboul angle、the volumefraction of necrotic area, Orthotopic typing method collapse risk prediction if the anteroposterior type are C1, C2 type. Conclusion Several prediction methods with advantages and disadvantages can not be applicated solely and should focus on orthotopic typing to make comprehensive analysis, early diagnosis is vital to improve the efficacy of Chinese medicine in preserving hip.
