胰腺实性假乳头状瘤临床病理分析

目的探讨胰腺实性假乳头状瘤(SPTP)的临床病理学及免疫组织化学特点。方法 10例SPTP作HE、免疫组织化学(EnVision法)染色观察。结果 10例SPTP中9例为女性,1例男性,年龄12～58岁,平均年龄29岁,术后随访均无复发。肿瘤体积较大,平均直径6.8cm,有包膜,囊实性相间。组织学上肿瘤由乳头和囊实区混合组成,细胞形态一致,核圆或卵圆,异型不明显,核分裂象罕见。瘤细胞围绕纤维血管轴心形成特征性假乳头结构。常见泡沫细胞团、胆固醇裂隙。免疫组织化学vimentin阳性(10/10),a-AT阳性(8/10),CD10阳性(10/10),CD56阳性(9/10),PR阳性(nuclear)(8/10),AE1/AE3阳性(1/10),Syn阳性(8/10),CgA阴性(0/10),EMA阴性(0/10)。结论胰腺SPTP好发于年轻女性,具有低度恶性潜能,免疫组织化学检测对其诊断和鉴别诊断具有较重要的价值,手术治疗对SPTP的疗效较好。

Clinicopathologic Study on Solid Pseudopapillary Tumor of the Pancreas
