清肝解毒汤联合更昔洛韦眼用凝胶治疗单纯疱疹病毒性角膜炎的疗效及预防复发作用

目的探讨清肝解毒汤联合更昔洛韦眼用凝胶治疗单纯疱疹病毒性角膜炎（HSK）的疗效及预防复发作用。方法选择68例HSK患者,随机分为观察组与对照组。两组患者均予以用0.15%更昔洛韦眼用凝胶滴眼,4次/d,每次2滴,观察组加用口服自清肝解毒汤,1剂/d,连用4周。对照组除不口服清肝解毒汤外余治疗同观察组。结果治疗4周后,观察组的总有效率明显高于对照组（χ2=6.48,P〈0.05）;且观察组的视力提高（≥1行）的总有效率明显高于对照组（χ2=6.35,P〈0.05）。两组中均有部分患者滴眼后出现轻微眼部疼痛不适,分别发生2例和4例,症状较轻,未发现严重的药物不良反应。两组患者不良反应发生率比较无明显差异（χ2=0.18,P〉0.05）。对治疗有效者治疗后随访6个月和1年,观察组复发率明显低于对照组（χ2=4.74、6.39,P〈0.05）。结论清肝解毒汤联合更昔洛韦眼用凝胶治疗HSK的疗效确切,能明显提高视力,安全性较好;且能明显降低HSK的复发率,具有治疗和预防HSK复发的作用。

Curative effect and preventive reoccurrence function of liver detoxification soup combined with ganciclovir ophthalmic gel on herpes simplex keratitis

Objective To discuss the curative effect and reoccurrence prevent function of liver detoxification soup combined with ganciclovir ophthalmic gel on herpes simplex keratitis（HSK）.Methods Sixty-eight cases of HSK patients were divided into observation group and control group randomly.The patients in two groups were given 0.15% ganciclovir ophthalmic gel dripped into the eyes 2 drips for one time and 4 times daily.The patients in observation group were given liver detoxification soup by the mouth.Except for liver detoxification soup,the patients in control group were given the same medical treatment as that in observation group.Results After 4 weeks＇ medical treatment,the total clinical efficiency of patients in observation group was much higher than that in control group（χ2=6.48,P0.05）,and the total efficiency of visual improvement（≥ one line） of patients in observation group was much higher than that in control group（χ2=6.35,P0.05）.Some patients in the two groups suffered light pain and discomfort of eyes after be dripping of ganciclovir ophthalmic gel into the eyes,and the numbers in observation group and control group are 2 and 4 cases separately,with light symptom but not serious drug adverse reaction（DAR）.After comparing the DAR occur rence rates during the medical treatment,no obvious statistical differences appeared（χ2=0.18,P0.05）.The follow-up of patients,who had be given effective treatment,was carried on after 6 month and 1 year of the medical treatment,and the reoccurrence rate in observation group was obviously lower than that in control group（χ2=4.74 and 6.39,P0.05）.Conclusion Liver detoxification soup combined with ganciclovir ophthalmic gel has reliable effect on HSK patients,which can improve the visual function obviously with high security,and lower the reoccurrence rate after the medical treatment with medical treatment and HSK prevention function as well.
