T2-3D-DRIVE与T1W-3D-FFE联合在血管压迫性面肌痉挛中的应用

目的探讨T2-3D-DRIVE与T1W-3D-FFE联合应用在血管压迫性面肌痉挛中的应用,并对面神经中枢段和外周段的断层方位选择进行研究。方法回顾性分析31例面肌痉挛患者的MR资料,术前均行T2-3DDRIVE与T1W-3D-FFE序列检查,分析颅内面神经与周围血管关系,并与手术结果相对照。结果 MRI发现面神经中枢段压迫或接触型27例。血管与面神经走行接近于垂直关系,脑池段接触型2例,两者走行接近于并行关系,无接触型2例。非痉挛侧中枢段接触型1例,呈垂直关系;脑池段接触型4例,呈并行关系3例,垂直关系1例。31例痉挛侧均行手术治疗,术中证实有血管压迫或接触面神经。结论 T2-3D-DRIVE能清晰显示面神经与周围血管的关系,而T1W-3D-FFE能辅助显示邻近血管的来源,两者结合可成为面神经与周围血管关系的最佳MR检查方案,另外,对于面神经中枢段的显示,横断位、斜冠状位基本可满足诊断的需要,而脑池段应加做斜矢状位的重建,提高判断的准确性。

The application of T2 -3D -DRIVE and T1W -3D -FFE combination in neurovascular compression in patients with hemifacial spasm

Objective To study the application of T2-3D-DRIVE and T1W-3D-FFE combination in neurovascular compression in patients with hemifacial spasm and research choose of the fault location about facial nerve center section and peripheral segment.Methods MR manifestation of 31 patients with hemifacial spasm was reviewed retrospectively.Before surgery,every patient were checked with T2-3D-DRIVE and T1W-3D-FFE squence.The relationship of intracranial facial nerve and around vessels was analyzed.Results MRI showed that there was facial nerve center section oppressed or contacted in 27,vessel and facial nerve goneline vertical,facial nerve root contacted in 2.Both were parallel to each other,no vessels contacted in 2.Non-centersection contacted in 1 and vertical,facial nerve root in 4 which 3 of them were paralleled and 1 of them were vertical.The 31 patient were all surgeried in which confirmed the vessels were oppressed or the facial nerve were contacted.Conclusion The T2-3D-DRIVE sequence can show the relationship between facial nerve and the around vessels clearly,but the T1W-3D-FFE can show the source of the adjacent blood vessels.The combination of them can tell the best check between the facial nerve and around vessels.Another,about the show of facial nerve center section,the axial position,oblique coronal position can meet the need of diagnosis.But the facial nerve root should make the sagittal,improve the accuracy of diagnosis.
