沟槽状胰腺炎6例分析

目的探讨沟槽状胰腺炎的诊断和治疗。方法回顾性分析1989年1月至2003年1月间在本院因疑为胰头癌而行胰十二指肠切除，术后病理证实为沟槽状胰腺炎的6例患者。结果本组6例患者中，临床表现包括波动性黄疸，上腹部隐痛不适、呕吐、体质量减轻，术前诊断为胰头癌；手术后，除1例死于术后并发症，1例失访外，余4例至今存活。结论沟槽状胰腺炎的临床表现和影像学检查均无特异性，超声内镜检查有助于诊断；诊断时，应注意与胰头癌相鉴别；不同的患者，可采用不同的治疗方法．对于严重者，保留幽门的胰头十二指肠切除术有较好的疗效。

Groove pancestitis: report of 6 cases

Objective To evaluate the diagnosis and treatment of groove pancreatitis. Methods The therapeutic result of 6 cases from January 1989 to January 2003 was reviewed retrospectively. These patients have undergone Whipple operation in the hospital because pancreatic head carcinoma was not ruled out. However, the histopathologic diagnosis was groove pancreatitis. Results In 6 cases, clinical manifestations included fluctuant jaundice,epigastric pain, vomiting, loss of body mass. Preoperative diagnosis was pancreatic head carcinoma. One patient died, one patient lost follow-up, the other 4 patients survived up to now. Conclusion The clinical manifestation and radiological features of groove pancreatitis are not specific.The way of edosography may help to diagnosis. This disease should be kept in mind as a differential diagnosis from pancreatic head carcinoma. For different patients, the different treatments are in selection. To serious patients, pylorus-preserving pancreaticoduodenectomy is a favorable alternative to Whipple operation from obstruction resulting from this disease.
