摘 要: | 目的 探讨窄谱中波紫外线(NB-UVB)联合复方甘草酸苷治疗斑秃的疗效及机制。方法 将73例斑秃患者随机分为联合组37例和药物组36例,另选37例健康查体者作为对照组。药物组及联合组均口服复方甘草酸苷50 mg,3次/d,联合组在此基础上行NB-UVB照射治疗,治疗3个月后判定两组疗效。采用流式细胞仪检测三组治疗前后外周血CD3^+、CD4^+、CD8^+及CD4^+/CD8^+细胞水平变化。结果 联合组及药物组有效率分别为91.9%、71.2%,P〈0.05。治疗前联合组及药物组CD3^+、CD4^+及CD4^+/CD8^+水平均明显低于对照组,P均〈0.05;治疗后联合组及药物组CD3^+、CD4^+及CD4^+/CD8^+均明显高于治疗前(P均〈0.05),CD8^+水平无明显变化;联合组CD3^+、CD4^+及CD4^+/CD8^+明显高于药物组(P〈0.05),CD8^+无明显变化。结论 斑秃患者免疫功能存在异常,NB-UVB联合复方甘草酸苷治疗斑秃疗效确切;其机制可能为改善患者的免疫功能。

Abstract: | Objective To investigate the therapeutic effect and mechanism of narrow-band ultraviolet ( NB-UVB ) combined with compound glycyrrhizin in the treatment of alopecia areata .Methods A total of 73 patients with alopecia areata were randomly divided into the drug treatment group (37 patients) and the combined treatment group (36 patients). A total of 37 patients were selected as health control group who did health examination in the same time .The drug treatment group and the combined treatment group were given oral compound glycyrrhizin at 50 mg, 3 times/d.The combined treatment group was given NB-UVB radiation therapy besides the oral compound glycyrrhizin .The treatment effectiveness for both groups was evaluated after 3 months.The change of the CD 3^+, CD4^+and CD4^+/CD8^+cellular level in peripheral blood was measured by flow cytometry before and after treatment .Results The effective rate of combined treatment group and drug treatment group was 91 .9%and 71 .2%, respectively .The effective rate of combined treatment group was higher than that of drug treatment group ( P〈0 .05 ) .Before treatment , compared with the health control group , the cellular level of CD3^+, CD4^+and CD4^+/CD8^+of the combine group and the drug treatment group was significantly lower (P〈0.05).After treatment , the cellular level of CD 3^+, CD4^+and CD4^+/CD8^+of both the combine group and the drug treatment group was significant higher than before treatment (P〈0.05).However, CD8^+level was no significant changed in combine group and drug treatment group before and after treatment .The change of cellular level of CD 3^+, CD4^+and CD4^+/CD8^+of the combine group was significantly higher than drug treatment group (P〈0.05).However, CD8^+level was no significant changed in combine group before and after treatment .Conclusions The dysfunction of immune system exists in patients with alopecia areata .NB-UVB combined with glycyrrhizin to treat alopecia areata proved to hav |
