LASIK手术前后高阶像差和对比、眩光敏感度的关系

摘    要:目的:研究近视准分子激光原位角膜磨镶术(LASIK)手术前后高阶像差和对比敏感度、眩光敏感度的关系,探讨LASIK手术前后高阶像差对视觉质量的影响。方法:对行LASIK手术的66例患者132眼(平均球镜度-5.57±2.30D,柱镜度-0.61±0.53D)分别测量术前和术后各期的高阶像差、对比敏感度和眩光敏感度,并进行相关性分析。结果:(1)术前,球差对12.0c/d频段的对比敏感度(CS)、眩光敏感度(GS)有影响。(2)术后1wk,彗差对12.0c/d和18.0c/d频段的CS、GS有影响;三叶草像差对6.0c/d、12.0c/d频段的CS和全频段的GS有影响;球差对6.0c/d、12.0c/d和18.0c/d频段的CS、GS有影响;高阶散光对全频段的CS、GS均有影响。(3)术后1mo,彗差对12.0c/d和18.0c/d频段的CS、GS有影响;球差对12.0c/d频段的CS、GS有影响。(4)术后3mo,彗差对12.0c/d频段的CS和GS有影响。结论:(1)术前,高阶像差对对比敏感度、眩光敏感度的影响以球差为主;术后中晚期则以彗差为主。(2)术前术后,高阶像差对对比敏感度、眩光敏感度的影响均以12.0c/d频段最为显著。

Abstract:AIM:To investigate the effect of higher-order aberration on visual quality by studying the relationship between higher-order aberration and contrast sensitivity and glare sensitivity in myopic eyes before and after laser in situkeratomileusis (LASIK).METHODS: Higher-order aberration, contrast sensitivity and glare sensitivity were measured in 66 patients (132 eyes, spheric diopter -5.57±2.30D, cylindric diopter -0.61±0.53D) by ARK-10000 wavefront analyzer, CSV-1000E and CSV-1000E with glare respectively.RESULTS: There was significant correlation between total spherical aberration and contrast sensitivity (CS) and glare sensitivity (GS) at 12.0c/d spatial frequency before LASIK. At postoperative 1 week, total coma was correlated with CS and GS at 12.0c/d and 18.0c/d spatial frequencies. Total trefoil aberration was correlated with CS at 6.0c/d and 12.0c/d and with GS at all frequencies. Total spherical aberration was correlated with CS and GS at 6.0c/d, 12.0c/d and 18.0c/d. And high astigmatism had effect on CS and GS at all frequencies. At postoperative 1 month, total coma aberration showed significant relationship with CS and GS at 12.0c/d and 18.0c/d. Total spherical aberration showed significant relationship with CS and GS at 12.0c/d. At 3 months after LASIK, there was only significant correlation between total coma aberration and CS and GS at 12.0c/d spatial frequency.CONCLUSION: The effect of higher-order aberration on CS and GS is mainly on total spherical aberration before LASIK, and on coma aberration after LASIK. The effect of higher-order aberration on CS and GS is most sensitive at 12.0c/d spatial frequency both before and after LASIK.
