肺类风湿病活动期的CT表现与肺功能相关性研究

目的探讨肺类风湿性病患者在类风湿活动期肺部CT表现及其与肺功能的相关性。方法回顾分析28例肺部有异常改变的肺类风湿性病活动期患者CT资料,定量分析肺内病变程度和范围并进行病变评分,且与肺功能的结果做相关性分析。结果28例肺类风湿性病活动期患者CT异常改变显示:①小叶间隔增厚16例,②不规则纤维索条影11例,③磨玻璃样变14例,④牵拉性细支气管扩张7例,⑤结节状气腔实变病灶4例,⑥蜂窝样灶3例,以网格样及磨玻璃样病变多见。2种表现的CT评分结果与肺弥散功能存在负相关性。结论CT视觉评分可以定量评估肺类风湿性病肺内病变,预示肺功能状况,评价预后。

A correlative study of CT findings and pulmonary function in patients with pulmonary involvement during the active phase of rheumatoid arthritis

Objective It is a correlative study of CT findings and pu lm onary function in patients with pulmonary involvement during the active phase of rheumatoid arthritis. Methods The CT images of 28 cases of pulmonary involvemen t during the active phase of rheumatoid arthritis were retrospectively analyzed. The pulmonary abnormalities revealed on CT images including distribution and ex tend of the lesions were quantitatively analyzed and scored. The correlation of CT scores with the results of pulmonary function tests was compared statisticall y. Results Interlobular septa thickening was shown in 16 cases; ground-glass opacification was revealed i n 14 cases; irregular lines were found in 11 cases; bronchiolectasis was noted i n 7 cases; micro nodules were demonstrated in 4 cases; and honey combing alterat ion was visualized in 3 cases. The abnormality most frequent seen on CT images w as reticular shadow and ground-glass opacification. CT visual score had a nega tive correlation with pulmonary function. Conclusion CT visual score provides qu antitative evaluation of the pulmonary involvement during the active phase of rh eumatoid arthritis, and also indicates the pulmonary function and prognosis as w ell.
