儿童浮膝损伤的治疗探讨

目的 探讨外科手术治疗儿童浮膝损伤的必要性和优越性，观察骨折愈合情况、治疗效果及有无并发症。方法 28例浮膝损伤的儿童患者，男19例，女9例，年龄6～12岁，采用一次性切开复位内固定或外固定器固定。随访18个月～7年，按Karlstrom的标准评定治疗结果。结果 本组病例均获随访，骨折全部一次愈合，未出现骨折不愈合及畸形愈合，2例出现患肢较对侧长1．2cm和1．5cm，3例短0．8～1．2cm，无膝关节功能严重障碍等并发症。优良率92．8％，治愈率71.4％。结论 对于5岁以上儿童浮膝损伤，一次性切开复位内固定或外固定器固定治疗股骨和胫骨骨折，有利于整体护理、早期功能恢复、缩短住院时间，且避免严重并发症发生。

Treatment of floating knee injury in children

Objective To evaluate the results of surgical treatment for floating knee injury in children. Methods Twenty-eight children (male 19, female 9) aged from 6 to 12 years with floating knee injury were managed with open reduction and internal fixation or external fixators in this hospital. All patients were followed up for 18 months to 7 years. The efficacy of the treatment was evaluated using Karlstrom criteria. Results There was no non-union or deformity in these patients. Discrepancy in the length of the limb was detected in 5 patients, two patients had longer and three patients had shorter limb than the controlateral side limb. The knee function was good in 92.8% of cases. Conclusions It might be recommended to treat children older than 5 years with floating knee injury with open reduction with internal fixation or external fixator.
