寰椎椎板钩枢椎椎弓根螺钉加钛缆治疗小儿寰枢椎脱位的疗效观察

摘    要:目的 评估后路寰椎椎板钩枢椎椎弓根螺钉加钛缆固定治疗小儿寰枢椎脱位的手术治疗方法,讨论小儿寰枢椎脱位的诊断及治疗.方法 对7例寰枢椎脱位伴齿突骨折的患儿行后路寰椎椎板钩枢椎椎弓根螺钉加髂骨植骨钛缆固定术.术前均行X线片、CT等影像学检查;术中在直视下行寰椎置椎板钩,枢椎椎弓根螺钉,钛棒连接,复位,髂骨植骨钛缆固定.术后颈托固定1个月.结果 所有螺钉均成功置入,复位固定满意,植骨全部融合,无鹅颈等发育性畸形出现.结论 后路寰椎椎板钩枢椎椎弓根螺钉加钛缆固定具有术中复位满意、固定牢靠、融合率高等特点,为寰枢椎脱位患儿治疗提供了一种较好的内固定术式.

Abstract:Objective To evaluate clinical outcome of C1 hook and C2 isthmic screws combined with titanium cable in the treatment of atlantoaxial dislocation in children and both diagnosis and treatment were discussed.Methods Seven children of atlantoaxial dislocation were analyzed retrospectively after they were treated with posterior iliac bone graft,fixation of C1 hook and C2 isthmic screws combined with titanium cable and postoperative cervical gear fixation for one month.Results All patients had bony fusion and no developmental abnormalilities such as gooseneck existed.Conclusions The fixation of C1 hook and C2 isthmic screws combined with titanium cable is easy and safe.It provides good fixation leading to high rate of graft fusion.It can be a good choice of the treatment of atlantoaxial dislocation in children.
