恶性肿瘤患者凝血指标的临床分析

目的 探讨恶性肿瘤患者是否存在凝血异常,分析导致异常的相关因素及临床意义.方法 对 2010年1月1日至10月31日就诊于山东省立医院的634例恶性肿瘤组、243例对照组患者的血浆凝血酶原时间(PT)、部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(Fib)、D-二聚体、血小板(PLT)进行检测.结果 恶性肿瘤组凝血异常率、PT、APTT、血浆Fib含量及D-二聚体阳性率均明显升高,差异有统计学意义(P<0.01);综合治疗组相比于初治组,血浆PT、APTT、D-二聚体阳性率明显升高,差异有统计学意义(P<0.01);血浆Fib含量在肺癌、食管癌患者中明显升高,差异有统计学意义(P<0.01).结论 恶性肿瘤患者PT、APTT延长,D-二聚体阳性率显著增加,是发生血栓性疾病的高危人群,对上述指标进行监测,有助于了解肿瘤进展,减少并发症的发生.

Clinical analysis of hemagglutination index in patients with malignant tumor

Objective To investigate the coagulation disorder in patients with malignant tumor,and analyse related factors, clinical significance. Methods The index （ PT, APTT, TT, Fib, D-dimer, PLT） of 634 hospitalized patients with malignant tumor and 243 patients as control in Provincial Hospital Affiliated to Shandong University from January 1,2010 to October 31,2010 were collected. Results Compared with control group, rate of the coagulation disorder, PT, APTT, Fib and D-dimer were significantly higher in tumor patients （ P 〈 0. 01 ） ; PT, APTT, D-dimer of post-treatment patients were significantly higher comparing to pre-treatment group （ P 〈 0. 01 ）. Fib were significantly higher in lung cancer and esophageal cancer patients than in other tumors （P 〈 0. 01 ）. Conclusions Therising of PT, APTT, D-dimer in tumor patients is a risk factor that lead to thrombotic disease, which should be properly monitored in order to reduce the complication occurrence.
