呼吸道异物420例临床分析

目的探讨呼吸道异物的诊断和治疗方法。方法回顾分析了1983年10月至2008年4月我科治疗的420例呼吸道异物的处理过程。结果3岁以下婴幼儿365例，约占87％；误诊27例，约占6．43％；420例（99．05％）在全麻或表麻下经直接喉镜、气管镜成功取出，2例死亡、2例脑轻瘫。结论全麻下支气管镜检及异物取出术安全，手术成功率高。对气管异物、支气管异物术前正确快速判断，术中需做好应变准备，减少并发症和降低病死率。

The Clinical Analysis of 420 Cases of Airway Foreign Bodies

Objective To detect the diagnosis and treatment means of airway foreign bodies. Method The clinical records of 424 patients with a history suggestive of foreign body aspiration from Oct. 1993 to Apr. 2008 have been reviewed. Result 365 eases （87%）were below 3 years old among of them and 27 eases （6.43%） were misdiagnosed as respiratory infections and asthma. Rigid bronehoscopies undergoing intravenous or topical anesthesia were performed in 420 patients and 420 foreign-bodies （FB） were found and extracted. Two patients died and two patients had a cerebral palsy after surgery .Conclusion Rigid bronchoscope combining with intravenous anesthesia is safer for tracheobronchial foreign body removal in children. Prompt and correct diagnosis are critieal in the treatment of foreign bodies in decreasing morbidity and mortality in trachea and bronchus
