| 摘 要： | 目的：报道髂骨复合瓣移植肢体运动障碍前瞻性临床研究结果。方法：３９例下颌骨缺损患者切取旋髂深动脉髂骨复合瓣４１块，随访半年以上２８例患者采用自拟运动功能评价百分法分优、良、可、差四个等级进行综合评价。结果：６个月：优５３．５７％（１５／２８），良１４．２９％（４／２８），可２１．４３％（６／２８），差１０．７１％（３／２８）；１年：优９０％（９／１０），差１０％（１／１０）。结论：①大多数患者运动障碍是暂时的，６个月～１年均能恢复至正常或接近正常。②股神经麻痹与术中过度剥离髂腰肌及臀肌而又未有效重建新的骨肌附丽是肢体运动障碍的两个主要原因。

| Abstract: | Objective:To report the prospective studies on limb dyskinesia following iliac crest composite free flap transplantation.Methods:According to the motor function hundred mark score,four grades of excellent,good,passable,bad were used to assess motor function.Results:6 months:excellent,53.57%;good,14.29%;passable,21.43%;bad,10%.1 year:excellent,90%;bad,10%.Conclusions:①Majority of patients with postoperative dyskinesia could recover during 6 months to 1 year after operation.②There are two main reasons for limb dyskinesia:femoral neuroparalysis;excessive stripping off but incomplete reattachment of the gluteus and iliopsoas. |
