生物反馈治疗慢性功能性便秘的临床研究

摘    要:目的评价生物反馈对慢性功能性便秘治疗的疗效。方法对门诊30例功能性便秘患者进行生物反馈治疗,治疗前后进行临床评估、结肠通过时间检查和肛门直肠压力测定。结果 出口梗阻型便秘患者临床症状改善,结肠通过时间较治疗前明显缩短(P<0.05),肛管直肠压力测定示直肠感觉阈值较治疗前降低,最大耐受量降低(P<0.05),肛门内、外括约肌的矛盾运动全部消失。慢传输型便秘患者结肠通过时间较治疗前明显缩短,肛管直肠测压示直肠感觉阈值较治疗前亦降低,最大耐受量降低,但无统计学意义(P>0.05)。结论生物反馈训练是一种治疗慢性功能性便秘行之有效、而且有确切长期疗效、无副作用的方法,值得在临床上广泛推广。

Abstract:Objective To assess the clinic effect of biofeedback treatment for different types of functional constipation. Methods 30 patients were enlisted to take the biofeedback training in the clinic. Assessing bowel symptoms, checking colonic transit time and anorectal manometry were made before and after the therapy. Results In the patients with outlet obstruction constipation (n = 20), the symptoms got improved, the colonic transit times decreased (P< 0. 05), the rectal volume sensory threshold and the maximum tolerance volume reduced (P<0.05). In the patients with slow transit constipation (n = 10), the symptoms also got improved,the colonic transit times decreased (P<0.05), the rectal volume sensory threshold and the maximum tolerance volume reduced only insignificantly (P>0.05). Conclusion Biofeedback treatment is effective for different forms of constipation, especially for outlet obstruction constipation. It can be one of the most important ways to cure functional constipation.
