肺转移瘤诊断及手术治疗对其预后的影响

目的探讨肺转移瘤的诊断及手术疗效。方法回顾性分析12例肺转移瘤手术治疗患者原发肿瘤的部位，肺转移瘤的大小、出现时间、部位、数目，转移灶的影像学表现，手术方法及疗效。结果本组原发恶性肿瘤包括消化系统恶性肿瘤6例，乳腺癌4例，泌尿系统肿瘤2例。肺转移瘤出现的时间平均为38．2个月。全组无手术死亡和发生严重并发症病例，术后生存时间达1年者10例（10／12）、3年者5例（5／12）、5年者3例（3／12）。结论对符合手术适应证的肺转移瘤患者，应积极进行外科手术治疗。能否正确诊断及彻底切除肺转移肿瘤是影响预后最重要的因素。

The Effects of the Diagnosis and Surgical on Prognosis of Pulmonary Metastases

Objective To study the diagnosis and surgical therapeutic effect of the pulmonary metastases tumor. Methods The location of the primary tumor, the size of pulmonary metastases tumor, the emergence time, site, numbers, imageology results and operation methods of metastasis tumors were analyzed on 12 patients with pulmonary metastases tumor, retrospectively. Results In the primary malignant tumors studied by us there are 6 digestive system tumors, 4 breast cancers and 2 urinary system tumors. The emergence time of pulmonary metastases tumor was difference, comparatively. There was no operative mortality or major complications. 1, 3, 5 -year survival rates were 83. 3%, 41. 7% and 25. 0%, respectively. Conclusion It indicated that the pulmonary metastases tumor patients with operation indication should be treated with surgery. The exactly diagnosis and complete resection of pulmonary metastases tumor is the most important factor for prognosis.
