早期乳腺癌术后背阔肌肌皮瓣联合一期假体置入乳房再造与美学塑形研究

目的探讨早期乳腺癌术后背阔肌肌皮瓣联合一期假体置入乳房再造与美学塑形的临床应用效果。方法选取本院2011年12月-2013年10月收治的乳腺癌患者100例。按照知情同意以及自主选择的原则将患者分为对照组和观察组。其中对照组患者进行常规乳腺癌改良根治术治疗，而观察组则采用全乳腺切除术及背阔肌肌皮瓣联合一期假体置人乳房再造与美学塑形治疗。观察两组术后并发症、随访生存率、局部复发率、远处转移率，并评价观察组的美学塑形效果。结果观察组患者治疗后，塑形效果总有效率为94．0％。两组术后并发症发生率差异无统计学意义（P〉0．05）；两组手术成功率、随访生存率均为100．0％，术后局部复发率以及远处转移率均为0。结论早期乳腺癌术后背阔肌肌皮瓣联合一期假体置入乳房再造与美学塑形是治疗乳腺癌的有效手段，既可以达到治疗乳腺癌的目的．又能够帮助患者维持完美外形，值得临床推广。

Breast reconstruction and aesthetic shaping research of latissimus dorsi flap combined with one-stage prosthesis implants after operation of early breast cancer

Objective To investigate clinical effect of breast reconstruction and aesthetic shaping of latissimus dorsi flap combined with one-stage prosthesis implants after early breast cancer surgery. Methods 100 cases of patients with breast cancer in our hospital from December 2011 to October 2013 were selected as research objects. In accordance with the principles of informed consent and independent choice, they were divided into control group and observation group. Conventional modified radical mastectomy were applied in the control group. Full mastectomy and latissimus dorsi flap combined with one-stage prosthesis implants were used in the observation group. Postoperative complications, follow-up survival rate, local recurrence, distant metastasis rate of both groups were observed and compared, and aesthetic shaping effect of observation group was also evaluated. Results After treatment, the total shaping efficiency of observation group reached 94.0%. Postoperative complications incidence of both groups showed similar proportions, with no significant difference （P〉O.05）. Operation success rate and follow-up survival rate of both groups all reached 100.0%. Local recurrence and distant metastasis rate were all 0. Conclusion Breast reconstruction and aesthetic shaping of latissimus dorsi flap combined with one-stage prosthesis implants after operation is an effective treatment mean. It not only can treat breast cancer, but also help maintain a perfect shape, which is worthy of clinical promotion.
