多层螺旋CT诊断梗阻性黄疸的临床价值

摘    要:目的 探讨多层螺旋CT诊断梗阻性黄疸的临床价值.方法 收集由手术病理或ERCP证实为143例梗阻性黄疸患者的影像资料,包括螺旋CT平扫和增强扫描、MPR及CPR技术重建图像等.根据术前影像诊断的结果将疾病类型分为良性病变组、恶性病变组,对梗阻部位及性质进行分析,并与手术病理对照.结果 多层螺旋CT定位诊断准确率为100.0%;定性诊断准确率:良性病变组为96.70%,恶性病变组为94.23%.结论 多层螺旋CT对梗阻性黄疸的病变显示清晰,结合增强扫描、MPR、CPR等图像后处理,能直观、立体地显示胆管梗阻的部位、程度以及病变所累及的范围和程度,对梗阻部位和性质的诊断有较高的临床价值.

Clinical value of multi-slice spiral CT in the diagnosis of obstructive jaundice

Abstract:Objective To explore the clinical value of multi-slice spiral CT in the diagnosis of obstructive jaundice. Methods Image data of 143 patients with obstructive jaundice confirmed by the surgical pathology or ERCP was collected,the data included plain and enhanced scan of spiral CT,MPR and CPR technique to reconstruct image etc.The disease type was divided into the benign and the malignant group according to the results of preoperative imag- ing diagnosis.The site of obstruction and nature were analyzed and compared with surgical pathology. Results The ac- curate rate of localization diagnosis with multi-slice spiral CT was 100%, the accuracy rate of qualitative diagnosis:be- nign lesion group was 96.70%,and malignant lesions group was 94.23%. Conclusion Multi-slice spiral CT can show the obstructive jaundice lesions clearly and it combined with enhanced scanning,image post-processing of MPR,CPR and other image processing which can show intuitively and three-dimensional the site,extent and lesion involved range and degree and has higher diagnostic value diagnosing position and nature of obstruction.
