腹腔镜胃癌根治术对患者凝血功能的影响

目的研究腹腔镜胃癌根治术对凝血功能的影响。方法将本院收治的28例接受腹腔镜胃癌根治术的胃癌患者作为研究组,另选取同期接受常规开腹胃癌根治术的32例患者作为对照组,对比2组患者凝血功能的变化。结果 2组各时间点凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)以及凝血酶原国际标准化值(INR)均无显著差异,但2组术后24 h的PT水平均较术前显著缩短,而术后0、24 h的APTT和INR与术前相比无显著差异。研究组术后0、24 h的纤维蛋白原(FIB)及D-二聚物(DD)水平均较术前显著升高,对照组术后24 h的FIB及术后0、24 h的DD水平较术前显著升高;研究组术后24 h的FIB水平及术后0、24 h的DD水平均显著高于对照组。结论腹腔镜和开腹胃癌根治术均可对患者的凝血功能造成明显影响,而腹腔镜手术的影响更为明显,应采取相应措施预防凝血功能异常导致的术后血栓形成。

Influence of radical resection of gastric cancer on coagulation function

Objective To study the influence of radical resection of laparoscopic gastric cancer on coagulation function.Methods Twenty-eight patients with gastric cancer who received radical resection in our hospital were selected as research group while another 32 patients who received laparotomic radical resection at the same time as control group.The changes of coagulation function of the two groups were compared.Results There were no significant difference in the levels of prothrombin time(PT),activated partial thromboplastin time(APTT) and prothrombin international normalized ratio(INR) between both groups at each time point,but the level of PT 24 h after operation was significantly shorter than that before operation,while there were no significant difference in the levels of APTT and INR between 0,24 h after operation and pre-operation.The levels of fibrinogen(FIB) and D-dimer(DD) in the research group 0 and 24 h after operation were significantly higher than that before operation.The levels of FIB 24 h after operation and DD 0,24 h after operation were significantly higher than that before operation.The levels of FIB 24 h after operation and DD 0,24 h after operation in the research group were significantly higher than that in the control group.Conclusion Laparoscopic and laparotomic gastric cancer radical resection both have an obvious influence on blood coagulation function,but laparoscopic surgery is more apparent.Corresponding measures should be taken to prevent postoperative thrombosis caused by dysfunction of blood coagulation.
