微创穿刺引流术联合中医辨证治疗中重型高血压脑出血临床研究

目的：观察微创穿刺引流术联合中医辨证治疗中重型高血压脑出血的临床疗效。方法：将90例中重型高血压脑出血患者按随机数字表法分为治疗组48例和对照组42例。对照组采用微创穿刺引流术治疗,治疗组在对照组治疗基础上加中医辨证治疗,连续治疗3个月,治疗后比较两组患者的临床疗效及相关并发症。结果：治疗组有效率为70.9%,远高于对照组有效率50.0%（χ2=11.052,P=0.024）;治疗前两组患者脑血肿、脑水肿体积、神经功能缺损评分比较无明显差异（P〉0.05）,治疗后均较治疗前明显改善（P〈0.05）,但同期比较治疗组改善更加明显,差异有统计学意义（P〈0.05）。结论：微创穿刺引流术联合中医辨证治疗中重型高血压脑出血可有效促进神经功能恢复,对改善患者近期预后有重要价值。

Clinical Study of Minimally Invasive Puncture Drainage Combined with TCM Differentiation Treating Moderate and Severe Hypertensive Intracerebral Hemorrhage

Objective：To observe the clinical efficacy of minimally invasive puncture drainage combined with TCM differentiation treating moderate and severe hypertensive intracerebral hemorrhage. Methods ：90 cases with moderate and severe hypertensive intracerebral hem- orrhage were randomly divided into two groups. The control group of 42 cases was given single minimally invasive puncture drainage, and the treatment group of 48 cases was treated with TCM differentiation based on the control group. 3 months after treatment, the clinical ef- ficacy and complications of two groups were compared. Results：The total effective rate of the treatment group was 70.9%, which was higher than that of the control group 50.0% （X2 -- 11. 052,P = 0. 024）. There was no evident difference for cephalophyma, encephalede- ma volume and neurologic impairment score between two groups before treatment（ P 〉0.05） ,which improved evidently compared with that before treatment （ P 〈 0.05 ）. But the improvement of the treatment group was more evident, which had statistical significance（ P 〈 0.05 ）. Conclusion ： Minimally invasive puncture drainage combined with TCM differentiation treating moderate and severe hypertensive intracerebral hemorrhage can evidently promote the recovery of neurological function,which has significant value to improve short-term prognosis of patients.
