咳嗽症状对慢性阻塞性肺疾病急性加重期患者心理和睡眠状况影响及干预研究

目的观察咳嗽症状对慢性阻塞性肺疾病急性加重期(acute exacerbation of chronicobstructive pulmonary disease,AECOPD)患者心理情绪和睡眠状况影响及干预的疗效。方法60例确诊为AECOPD的住院患者随机分为观察组和对照组各30例。观察组男23例,女7例,年龄(73±5)岁;对照组男24例,女6例,年龄(74±3)岁。观察组口服复方磷酸可待因溶液,对照组口服复方甘草合剂进行干预治疗。在干预措施实施前后分别应用心理情绪量表(HAD)和睡眠状况量表(SRSS)对两组患者作心理症状和睡眠状况的测评以及咳嗽、痰量、痰液黏稠度和喘息症状的评分比较。结果干预前两组患者的心理症状、睡眠症状和咳嗽、痰量、痰液黏稠度和喘息症状的评估,其差异均无显著性(P>0.05)。干预后观察组的上述评定指标均明显优于对照组(P<0.005)。相关分析表明,咳嗽症状分值与心理症状的A评分D评分和睡眠状况总评分均呈显著性正相关(P<0.001)。结论应用复方磷酸可待因溶液干预治疗,可缓解AECOPD患者咳嗽、咳痰、喘息症状同时还可改善患者的心理症状和睡眠状况。

The influence of cough on psychology symptoms and sleep statuses and intervention effects in acute exacerbation patients of chronic obstructive pulmonary disease

Objective To investigate the influence of cough on psychology symptoms and sleep statuses and the intervention effects in acute exacerbation patients of chronic obstructive pulmonary disease(AECOPD).Methods Sixty inpatients of AECOPD were divided into observation group and control group rondomly(30 cases in each group).Two groups were treated by compound codeine phosphate solution(observation group) and compound liquorice mixture(control group).Psychology symptoms and sleep statuses were measured by hospital anxety and depression score(HAD) questionnaire and self rating sleepiness state(SRSS) questionnaire respectively and cough,sputum number,sputum thick degree and breathing were compaired before and after treatment.Results Psychology systems,sleep statuses,cough sputum number,sputum shick degree and breathing in observation group were remarkable improved after treatment and were better than those in control group(P<(0.005)).There were marked relatively between cough score and A score,D score and total sleep score(P<(0.001)).Conclusions Compound codeine phosphate solution could relax cough symptoms and improve psychology symptoms and steep statuses in AECOPD patients.
