循经取穴针推结合治疗急性腰部软组织损伤的临床疗效观察与机制分析

目的：比较针刺结合推拿与口服双氯芬酸钠胶囊对急性腰部软组织损伤患者腰痛程度和功能障碍的影响。方法130例患者随机分成治疗组和对照组，用疼痛目测类比定法（ VAS）和改良中文版Roland-Morris功能障碍问卷表（ CRMDQ）评价两组患者治疗前后指标变化。结果治疗组和对照组的总有效率、即刻疗效和1周时疗效差异有显著性（P〈0.01），两组间比较差异有显著性（P〈0.01）。治疗组VAS和CRM-DQ即刻和7天治疗前后自身对照差异有显著性（P〈0.01），对照组VAS和CRMDQ7天后自身对照差异有显著性（P〈0.01）。结论针刺结合推拿在治疗急性腰部软组织患者疼痛和改善腰部功能上具有明显优势。

Clinical Observation and Mechanism Analysis of Treating Acute Lumbar Soft Tissue Injury by Acupuncture combined with Tuina (Massage) according to the Acupoints on the Meridi-ans

Objective To compare the impact on lumbago degree and disability limited extents of acute lumbar soft tissue injury treating by acupuncture combined with Tuina or taking diclofenac sodium capsules. Method 130 patients were randomly divided into 2 groups： treatment group treated by acupuncture combined with Tuina, and control group treated by taking diclofenac sodium capsule. Lumbago degree and disability limited extents were evalu-ated by Visual Analogue Scale （ VAS） and Chinese vesion of Roland-Morris disability questionnaire （ CMRDQ） be-fore and after treatment. Result There is a significant difference in total effective rate, immediate effect and 1 week effect （P〈0. 01） before and after treatment. There is also a significant difference between the 2 groups （P〈0. 01）. In treatment group, it has a significant difference in immediate effect and 1 week effect evaluated by VAS and CRM-DQ before and after treatment （P〈0. 01）. In control group, it has a significant difference in immediate effect and 1 week effect evaluated by VAS and CRMDQ before and after treatment （P〈0. 01）. Conclusion Acupuncture and Tuina have an obvious advantage in relieving pain and improve lumbar function of acute lumbar soft tissue injure.
