摘    要:目的 应用速度向量成像(VVI)技术定量分析妊娠高血压综合征(妊高征)孕妇胎儿左室心肌力学,寻找评价胎儿心肌力学最有价值的指标。方法 妊高征组孕妇36例,对照组孕妇54例。应用VVI技术评价胎儿左室心肌收缩期和舒张期6个节段及整体峰值速度、应变、应变率,评价妊高征孕妇胎儿心肌力学的特点,并与对照组参数进行比较。结果 ① 妊高征组胎儿左室收缩期和舒张期节段峰值速度、应变和应变率小于对照组,部分节段差异有统计学意义(P<0.05);②妊高征组胎儿左室收缩期和舒张期整体峰值速度、应变和应变率均小于正常对照组(P<0.05)。结论 VVI技术能敏感地反映妊高征孕妇胎儿心肌力学的变化。与节段应变和应变率比较,整体峰值应变和应变率更加敏感,将有望成为评价胎儿心肌力学最有价值的指标之一。

Abstract:Objective To explore the most valuable parameters for fetal myocardial mechanics by evaluating fetal left ventricular myocardial mechanics in women with pregnancy-induced hypertension syndrome using velocity vector imaging (VVI). Methods The study involved two groups of pregnant women: the hypertensive group (women with pregnancy-induced hypertension, n=36) and control group (healthy subjects, n=54). The ventricular systolic and diastolic segmental and global peak velocity, strain, and strain rate were evaluated with VVI and compared between the two groups. Results Systolic and diastolic segmental peak velocity, strain and strain rate of the hypertensive group were lower than those of the control group. There were statistical differences in some of the segments (P<0.05). ② Systolic and diastolic global peak velocity, strain and strain rate of the hypertensive group were lower than those of the control group (P<0.05). Conclusion VVI can sensitively reflect the fetal myocardial mechanics in women withpregnancy-induced hypertension syndrome. Global strain and strain rate may be more sensitive than segmental strain and strain rate in the assessment of fetal myocardial mechanics.
