慢性阻塞性肺病体重指数与肺功能的相关性研究

摘    要:目的通过比较低体重指数和正常体重指数慢性阻塞性肺病(COPD)患者的肺功能、血气、呼吸困难评分、运动能力和呼吸肌肉功能,探讨COPD患者体重指数和肺功能的相关性。方法选稳定期中重度COPD患者为研究对象,均为男性,以体重指数(BMI)≤18.5kg/m^2者为低体重指数组和30〉BMI〉18.5kg/m^2为正常体重指数组;比较两组肺功能、血气、6分钟步行距离和最大吸气压和最大呼吸压,分析BMI和上述各项指标的相关性。结果低体重组21例,正常体重组19例,两组肺功能、血气、呼吸困难评分、6分钟步行距离无统计学差异;最大呼气气压两组无统计学差异,但最大吸气压正常体重指数组高于低体重指数组,有统计学差异(P〈0.05);相关性分析表明除最大吸气压与BMI有较强的相关性外(相关系数0.301,P〈0.05),其余各指标与BMI相关性均无统计学差异。结论COPD患者的体重指数和最大吸气压可能有一定的相关性。

Abstract:Objective To initially investigate the correlation between body mass index (BMI) and pulmonary function through comparison of pulmonary function, arterial blood gas, severity of dyspnea, exercise capacity and power of respiratory muscle between lower BMI COPD patients and normal BMI COPD patients. Methods The moderate and severe COPD patients in the stable stage were recruited in our study, all of them were male. Based on BMI, we divided the patients into the lower BMI group (BMI≤18.5 kg/m^2) and the normal BMI group (30 〉 BMI 〉 18.5 kg/m^2), and then, compared the pulmonary function, arterial blood gas, severity of dyspnea, 6 minutes walking test (6 MWT), maximal inspiration pressure (MIP) and maximal expiration pressure (MEP). The correlations between BMI and the above mentioned parameters between the two groups were conducted. Results Uhimatedly, there were 21 patients in the lower BMI group and 19 patients in the normal BMI groups. There were no significant differences in pulmonary function, arterial blood gas, severity of dyspnea, 6 MWT, severity of dyspnea, between the two groups. MIP were greater in the normal BMI group than in the lower BMI group, but MEP showed no group difference (P 〉 0. 05 ). Correlation analysis demonstrated that BMI was strongly correlated with MIP (correlation coefficient was 0. 301, P 〈 0.05 ). No statistical correlation was found among other parameters. Conclusion BMI may have more correlation with MIP in COPD patients than any other parameters.
