出血坏死性鼻息肉的CT和MRI诊断价值

目的探讨CT和MRI对出血坏死性鼻息肉的诊断价值。方法回顾性分析21例经手术病理证实的出血坏死性鼻息肉的影像学资料，21例行CT平扫，10例行MRI平扫，其中4例同时行MRI增强检查。结果21例病变均起源于上颌窦，形态多样。12例CT平扫表现为密度不均匀的软组织包块，其中7例病变内部及边缘可见斑片状高密度影；20例病变周围骨质压迫吸收，以上颌窦内侧壁为著。10例MRI平扫表现为长T1、长T2信号，内部及周围见短T2信号；4例MRI增强后呈斑片状、结节状不均匀明显强化。结论MRI与CT比较能更好地显示出血坏死性鼻息肉出血、坏死的信号特点，有助于病变的定性诊断。

THE VALUE OF CT AND MRI IN DIAGNOSIS OF HEMORRHAGIC AND NECROTIC NASAL POLYPS

Objective To explore the value of CT and MRI in the diagnosis of hemorrhagic and necrotic nasal polyps. Methods Twenty-one patients with hemorrhagic and necrotic nasal polyps were reviewed retrospectively. who were confirmed by operation and pathology. CT scan was performed in all the 21 casesv lt） cases underwent MRI scan. in which four received contrastenhanced MRI scan. Results All the 21 lesions originated from the maxillary sinus with variable shapes. On CT .12 cases lesions showed inhomogeneous soft tissue. and the patchy-like hyperdense was observed in the edge and center of seven cases. The involved bones of the lesions were compressed and absorbed in 20 cases. especially the inner wall of the maxillary sinus. On MRI.10 lesions demonstrated hypointensity on T1 WI and hyperintensity on T2WI with hypo intensity in the edge and inside. Post-contrast MRI revealed marked patch and nodular heterogeneous enhancement in four cases. Conclusion MRI is better than CT to show the signal features of bleeding and necrosis of hemorrhagic and necrotic nasal polyps and conduce to making a qualitative diagnosis.
