口内牙片数字影像软件在牙种植设计中的应用

摘    要:目的应用口内牙片数字影像软件(DFW)测量并分析全口牙位曲面体层X线影像的相关数据,设计牙种植方案。方法将基准标记杆置于60例(76颗)牙列缺损或缺失病人口腔内,拍全口牙位曲面体层X线片,并将其导入DFW中;用电子数显卡尺精确测量基准标记杆长度;将全口牙位曲面体层X线影像在DFW校准页窗口打开,选择图像校准功能,自基准标记杆的一端向另一端画一校准线,在真实距离栏输入用数显卡尺测量得到的基准标记杆长度,点击校准结束按钮完成图像校正;在校准后的全口牙位曲面体层影像上标记下牙槽神经管、上颌窦底等重要解剖结构的位置,测量牙槽嵴顶高度、缺牙间隙宽度,确定备用种植体的长度和粗度,完成牙种植方案设计。术后按照术前相同的拍摄方法和条件即刻拍摄全口牙位曲面体层X线片,将其导入DFW中进行处理,测量影像中的种植体长度,与种植体的真实长度进行比较。结果种植体均顺利按预期设计植入至病人口腔内,种植体术后影像中长度为(10.51±1.36)mm,真实长度为(10.50±1.36)mm,差异无显著性(P〉0.05)。结论应用DFW对全口牙位曲面体层X线图像进行校正、测量,能够为种植体选择提供精确的数据。

APPLICATION OF DIGORA DIGITAL IMAGING SYSTEM IN DENTAL IMPLANTATION DESIGN

Abstract:Objective To measure and analyze the correlated data of orthopantomogram using diagnostics for Windows(DFW)software and design a program for dental implantation. Methods A marking rod was placed in oral cavity of 60patients(76teeth)with defect or absence of dentition,an orthopantomogram was taken and inputted into DFW.The length of the marking rod was accurately measured by electronic digital calipers(EDC).The panoramic radiographs were opened through DFW calibration window,and the calibration line was selected and drawn from one end of the marking rod to the other end.The length of the rod measured by the EDC was then inputted into the actual distance column,and clicked the button to end the image correction.The locations of important anatomic structures-such as the canal of inferior alveolar nerve and the base of maxillary sinus-were marked,the height of alveolar ridge crest and the width of edentulous space were measured,the length and diameter of ready-for-use implants were determined,the dental implantation design thus completed.After surgery,orthopantomograms were taken-under the same condition and method as done before surgery-and entered DFW for handling,measuring the length of the implants on the image,and comparing with its true length. Results The implants were smoothly inserted into oral cavity of the patients as expected.The average length of the implants on image was(10.51±1.36)mm,and its real average length was(10.50±1.36)mm,with no statistically significant difference was noted(P〈0.05). Conclusion Employing DFW software to revise and measure the image of orthopantomogram can provide accurate data for selecting implants.
