高褶虹膜构型在原发性闭角型青光眼中的发病情况

摘    要:目的通过探讨高褶虹膜构型在原发性闭角型青光眼(PACG)中的构成情况,以明确高褶虹膜构型对PACG的临床病程是否有影响。方法应用超声生物显微镜(UBM)检测138只PACG的眼前节结构,其中慢性闭角型青光眼98只,急性闭角型青光眼40只。每例均扫描眼前节上方、下方、鼻侧、颞侧4个象限。采用高褶虹膜的诊断标准,至少2个象限符合所有条件才可诊断为高褶虹膜构型。结果共有30只眼诊断为高褶虹膜构型,其中慢性闭角型青光眼组24只眼、急性闭角型青光眼组6只眼,两组比较差异无显著性(P〉0.05)。结论高褶虹膜是PACG重要的发病机制,但是PACG临床表现为急性或慢性与是否存在高褶虹膜构型无明显相关性。

Abstract:Objective To identify whether plateau iris configuration(PIC)can impact the clinical course of primary angle-closure glaucoma(PACG)by investigating the prevalence of PIC in PACG. Methods Using ultrasound biomicroscope(UBM)to detect the anterior segment structures of 138eyes with PACG including 98eyes with primary chronic angle-closure glaucoma(PCACG)and 40eyes with primary acute angle-closure glaucoma(PAACG).Radial scans were carried out in all four quadrants-inferior,superior,nasal and temporal.Adopting the plateau iris diagnostic criteria,eyes with PIC were defined as those with at least two quadrants met all of these criteria. Results PIC was found in 30of 138eyes with PACG,of which,24eyes with PCACG and six with PAACG.No significant difference in the prevalence of PIC was noted between the two groups(P〉0.05).Conclusion Plateau iris is one important pathogenesis in PACG.There is no significant correlation between the manifestations of acute or chronic primary angle-closure glaucoma and plateau iris configuration.
