延髓背外侧综合征的临床和磁共振成像分析

目的探讨延髓背外侧综合征的临床表现与磁共振成像(MRI)的关系。方法分析2j例延髓背外侧综合征患者的临床资料及头颅MRI检查结果。结果本组部分患者有共济失调、眩晕、头晕、恶心、呕吐、吞咽困难、声音嘶哑、Horner征、面部感觉障碍、偏身感觉障碍。MRI检查结果显示,延髓上部损害是经典的斜带型,常伴随严重的吞咽困难、声音嘶哑和面瘫；而下部的损害常位于延髓外侧的表面,表现为明显的眩晕、眼震和共济失调。结论延髓背外侧综合征的临床特点与MRI的受累部位密切相关。充分认识这一点,对于尽快给予恰当的诊断、治疗及改善预后有所帮助。

The clinical and magnetic resonance imaging analysis of lateral medullary syndrome

Objective To investigate the relationship between clinical findings and magnetic resonance imag- ings(MRI)of lateral medullary syndrome.Method 25 patients suffered from with lateral medulary syndrome were analyzed with their clinical findings and MRI features.Results Gait ataxia(85%),Vertigo,dizziness (93%),nausea(71%),dysphasia(63%),hoarseness(51%),Horner sign(71%),facial (86%)and hemi body (93%)sensory changes were the frequent clinical findings.MRI features showed that the lesions located in rostral part of the medulla were usually in diagonal hand shaped and associated with more severe dysphasia,hoarseness and presence of facial paresis.Whereas the caudal lesions situated usually in the lateral surface of the medulla,appeared with more marked vertigo,nystagmus and gait ataxia.Conclusions The clinical characteristics of lateral medullary syndrome is closely correlated with the location of the lesion.This conclusion may help us make appropriate diagno- sis and treatment with improvement of the prognosis.
