局部注射IL-2联合化疗对口腔鳞癌浸润淋巴细胞的影响

目的　评价IL-2局部注射联合化疗对口腔鳞癌局部免疫反应的影响。方法　34例T3、T4期口腔鳞癌患者随机分成2组接受术前治疗,其中23例行免疫化疗(IL-2局部注射合并PVP化疗),11例行单纯化疗。比较 2组患者治疗前后肿瘤浸润T淋巴细胞亚群和B淋巴细胞的变化。结果　免疫化疗组治疗前后CD4+,CD8+, CD20+相对值分别为36·96,35·65,28·65和56·61,38·52,38·70。治疗后CD4+,CD20+细胞数较治疗前明显增加 (P<0·01)。单纯化疗组治疗前后CD4+,CD8+,CD20+细胞数均无显著差异(P>0·05)。结论　IL-2局部注射联合化疗对增强口腔鳞癌患者局部免疫功能具有重要意义。

Effect of Local Immunotherapy of Interleukin 2(IL-2) in Combination with Chemotherapy upon Intratumoral Lymphocytes in Oral Squamous Cell Carcinomas

Objective To evaluate the effect of locoregional immunotherapy of interleukin 2 (IL-2) in combination with chemotherapy upon intratumoral lymphocytes in oral squamous cell carcinomas.Methods Thirty-four patients with stage T3 or T4 oral squamous cell carcinoma were randomly divided into two groups, and treated with two therapies. 23 cases of them received immunochemotherapy and 11 cases received PVP chemotherapy. Changes of T lymphocyte subsets and B cells at tumor site in the two groups were compared before and after therapy.Results The relative numbers of CD4 ,CD8 ,CD20 before and after treatment in immunochemotherapy were respectively 36.96, 35.65, 28.65 and 56.61,38.52,38.70. The numbers of CD4 ,CD20 increased significantly after immunochemotherapy. However, in chemotherapy group, there was no significant difference in numbers of CD4 ,CD8 and CD20 cells between pre and post treatment.Conclusion Immunochemotherapy for oral squamous cell carcinomas may play an important role in increasing local immunity.
