摘    要:①目的评价人工膝关节置换术的临床疗效。②方法全膝关节置换术88例(105膝),男10例,女78例;平均年龄65.5岁(50~82岁);双侧同时置换17例。原发病:类风湿性关节炎6例(9膝),创伤性关节炎1例,骨性关节炎81例(95膝)。手术安放保留后交叉假体18膝,后方稳定性假体62膝,活动衬垫假体25膝。平均随访42月(12~108月),应用HSS膝关节评分系统进行疗效评定。③结果优90膝(85.71%),良10膝(9.52%),可3膝(2.86%),差2膝(1.90%),优良率95.23%。④结论人工全膝关节置换术能有效缓解膝关节疼痛,改善膝关节功能。正确地选择假体、精细的手术操作和严格的术后康复是保证手术效果的关键。

Abstract:Objective To estimate the results of the total knee replacement. Methods Eighty-eight patients (10 males and 78 females, 105 knees) were selected for total knee replacement. The average age was 65.5 years (range: 50-82). Bilateral knee replacement was carried out in 17 cases, in which, rheumatoid arthritis, six cases (9 knees); traumatic arthritis, one case; osteoarthritis, 81 cases (95 knees). The posterior cruciate ligament retaining prosthesis was done in 18 knees, posterior stabilizing prosthesis in 62 and mobile bearing prosthesis in 25. All the patients were followed up with an average of 42 months (12-108 months). Results According to HSS system, 90 knees (85.71%) had an excellent results; 10 knees (9.52%), good; three knees (2.86%), fair ; and two knees (1.90%), poor. Excellent and good rate was 95.23%. Conclusion Total knee replacement can decrease knee pain and improve its function. The selection of the prosthesis, surgical technique and postoperative rehabilitation are keys to the treatment outcome.
