子宫动脉化疗栓塞术治疗剖宫产瘢痕妊娠

目的探讨子宫动脉灌注化疗栓塞术治疗剖宫产瘢痕妊娠（CSP）的可行性和安全性。方法 2008年10月—2010年10月对12例CSP患者行子宫动脉化疗栓塞术,先向双侧子宫动脉灌注甲氨蝶呤80～100mg,再用500～710μmPVA颗粒对双侧子宫动脉进行栓塞,术后定期复查血清β-HCG及妇科超声。结果子宫动脉化疗栓塞术操作成功率100%,术后血清β-HCG水平迅速下降,妊娠囊逐渐萎缩、坏死、脱落。无大出血及子宫破裂等严重并发症。结论子宫动脉灌注化疗栓塞术治疗CSP能迅速止血,有效杀胚,保留子宫,是一种安全、可行的治疗方法。

Uterine artery chemoembolization in treating cesarean scar pregnancy

Objective To explore the feasibility and safety of uterine artery chemoembolization in the treatment of cesarean scar pregnancy(CSP).Methods From Oct 2008 to Oct 2010,uterine artery chemoemboliztion was performed in 12 patients with CSP.Firstly 80—100 mg methotrexate was infused into bilateral uterine arteries,and then the bilateral uterine arteries were embolized with 500—710 μm PVA particles.Serum β-human chorionic gonadotropin(β-HCG) level and gynecology ultrasound after the procedure were observed.Results The technical success rate of bilateral uterine artery chemoembolization was 100%.After operation the serum β-HCG level rapidly descended and the gestational sac gradually atrophied with necrosis and even desquamate.No serious complications occurred.Conclusion Uterine artery chemoembolization is an effective and safe treatment for CSP,which can stanch speedly,kill embryo effectively and preserve the patient’s fertility function.
