急性心肌梗死溶栓治疗ST-T变化的临床意义

目的 探讨急性心肌梗死（AMI）静脉溶栓后ST－T变化的意义。方法 57例静脉溶栓治疗病人，根据其溶栓后2h内心电图ST段下移程度分为恢复组（＞75％）、部分恢复组（25％－75％）、未恢复组（＜25％）。24h内T波是否倒置分为早期倒置组与延迟倒置组。并与冠脉间接再通指征比较。结果 冠脉再通组与ST段恢复组、部分恢复组均伴有早期T波倒置（P＜0．01），而未通且与ST段未恢复组则无早期T波倒置（P＜0．01）。结论 ST段恢复伴早期T波倒置对判断冠脉再通有一定价值。

Clinical significance of the variation of S-T segment and T wave after intravenous thrombolytic therapy for patients with acute myocardial infarction

Objective To investigate the clinical significance of the variation of S T and T wave after intravenous thrombolytic therapy for AMI patients.Methods The electrocardiograms of 57 patients with AMI treated by intravenous thrombolytic therapy were observed. According to the descending degree of S T segment in 2 hours after thrombolysis, they were divided into three groups: recovery group(more than 75%),partial recovery group(25% to 75%) and poor effect group(less than 25%).Meanwhile they were divided into early inverted group and delayed inverted group according to whether T wave would invert in 24 hours after thrombolysis. The variation of T wave and recovery degree of S T segment were compared with the indirect indication of coronary artery recanalization.Results The recanalizing group,S T segment recovery group and partial recovery group were all accompanied by early inverted T waves(P<0.01), which did not exist in the irrecanalizing group and S T segment abnormal group(P<0.01).Conclusion Recovery of S T segment with early inverted T waves has some value to judge coronary artery recanalization.
