皮层下脑血管病后言语障碍的康复和预后

目的：了解皮层下脑血管病的言语障碍的预后及言语康复治疗的效果。方法：《汉语失语症测评法》评价家庭中言语康复训练对44例皮层下脑血管病的言语障碍预后的影响。结果：32例皮层下失语症中81．25％在言语康复治疗3个月后好转，34．78％，在3个月时大致恢复正常，皮层下失语症病灶位于右侧者预后好于左侧者（1，3月好转率比较，P＜0．01），病灶为出血者好于梗塞者（1，3个月好转率比较，P＜0．01），12例经典失语症中，除完全性失语症恢复较差外，其余各型失语症恢复均较好。结论：家庭言语康复治疗实际可行，皮层下脑血管病患者言 障碍经语言康复治疗预后较好。

Rehabilitation and prognosis of speech disorder after subcortical cerebral vascular disease

Objective:To explore the prognostic of speech disorder after subcortical cerebral vascular disease(SCVD) and the role of family speech treatment on the speech disorder.Method:To scale 44 cases with speech disorder after SCVD in the procedure of family speech treatment with Chinese Aphasia Measurement Method.Result:Of 32 cases with subcortical aphasia syndrome (SCA),81.25% showed symptom improvement and 34.78% recovered to roughly normal level after 3 months family speech treatment.The prognostic of cases with lesion in right hemisphere was better than left (the difference of improvement rates was significant in time of 1 and 3 month, P <0.01).The prognosis of hemorrhage was better than that of infarction (the difference of improvement rates was significant in time of 1 and 3 month, P <0.01).12 cases with classical aphasia syndrome,apart from the global aphasia,have satisfactory recovery.Conclusion:The prognosis of speech disorder after SCVD is satisfactory after speech treatment and speech treatment, family treatment is a practical rehabilitation form.
