儿童肾小球肾炎的进行性损害与肾小管间质病变关系的探讨

目的探讨肾小管间质损害在儿童肾小球肾炎进展中的临床及病理意义.方法选择44例肾小球肾炎患儿,对其临床及病理进行分析.结果肾小管间质损害轻度15例,中度20例,重度9例.随着肾小管间质损害程度的加重,患儿血压升高,尿蛋白定量增加,肾小球及肾小管功能减退.肾小管间质损害程度与血肌酐呈正相关;肾小球和血管的病变也加重,球性硬化加重;肾小管间质损害程度与球性硬化呈正相关.结论肾小管间质损害可能是决定儿童肾小球疾病进行性发展的关键因素之一.

A study of the relationship between renal tubulointerstitial lesion and the progression of glomerular diseases in children

Objective To observe the change of renal tubulointerstitial cell and to analyse the characteristics of tubulointerstitial lesions(TIL) in children glomerulonephritis. Methods The pathologic and clinic data of 44 cases were analyzed. Results Mild TIL was found in 15 cases, moderate TIL in 20 cases and severe TIL in 9 cases. With the severity of TIL, the blood pressure and urine protein excretion were increased while the glomerular and tubular function were decreased. The severity of TIL damage was proportioned to Scr. The more severe the TIL, the more severe the glomerular and tubular damage and glomerular sclerosis. The TIL severity was positively related to the severity of glomerular sclerosis. Conclusions TIL may be a crucial factor in the progression of glomerular damage in children glomerular diseases.
