高血压左心室重构的超声心动图特征

摘    要:目的:观察高血压患者心脏形态、结构,认识左心室重构、类型及特点.方法:对240例高血压患者和60例正常人进行超声心动图检查,按照Ganau的方法判断左心室重构及构型分型,进行统计分析.结果:高血压组中左室正常构型者22.50%,向心性重构者36.25%,向心性肥厚者32.50%,离心性肥厚者8.75%.离心性肥厚者的心指数和每搏指数高于对照组和向心性重构者;射血分数和左室短轴缩短率向心性重构者增高,离心性肥厚患者值低;高血压组E/A比值降低,尤其是向心性肥厚明显.结论:高血压患者发生左心室重构,有不同几何构型和特点.

Abstract:Objective: To observe the cardiac structure,characterization of remodeling of left ventricle and geometric patterns of the patients with essential hypertension.Methods: 240 cases of the patients with essential hypertension were examined by echocardiography compared with 60 healthy subjects according to methods used by other specialists.Results: There was remodeling of left ventricle in the hypertension,the patterns of remolding were normal,concentric remolding,concentric hypertrophy,eccentric hypertrophy,which accounted for 22.50%? 6.25%? 32.50% and 8.75% respectively.The cardiac index,stroke volume index,ejection fraction,fraction of shortening,E/A ratio and so on were abnormal in the patients with hypertension,which correlated with different geometric patterns respectively.Conclusions: Remodeling of left ventricle occurs in hypertension,which has different geometric patterns and impairs heart functions.
