1例先天性乳糜胸早产儿的药物治疗与肠外营养监护

目的:探讨药师对先天性乳糜胸的药学监护要点。方法:临床药师参与1例保守治疗无效的先天性乳糜胸早产儿的治疗，针对治疗药物选择及用药监护提出意见，并结合先天性乳糜胸的临床特点，协助医师制定个体化肠外营养支持方案。结果:临床药师利用专业知识，依托循证医学证据提供合理用药建议，被临床医师采纳，使患儿得到有效治疗，避免了水电解质紊乱及并发症的发生。患儿经治疗后顺利出院。结论:临床药师参与新生儿先天性乳糜胸的治疗过程，开展药学服务，可有效促进患儿合理用药。

Pharmaceutical Care for Drug Therapy and Parenteral Nutrition in a Preterm Infant with Congenital Chylothorax

Objective: To explore the key points of pharmaceutical care for congenital chylothorax by pharmacists. Methods: Clinical pharmacists participated into the treatment of a preterm infant of congenital chylothorax with failed conservative treatment, provided suggestions on the selection and monitoring of medication of congenital chylothorax, and assisted the clinicians to develop individualized parenteral nutrition scheme based on the clinical characteristics of congenital chylothorax. Results: Relying on professional knowledge and evidence-based medical evidence, clinical pharmacists provided rational drug use suggestions, which were adopted by clinicians. The child received effective treatment, the water and electrolyte disorders and complications were avoided. The child was successfully discharged after treatment. Conclusion: Clinical pharmacists can promote rational drug use by participating into the treatment and conducting pharmaceutical care for infants with congenital chylothorax.
