手术结合针刺治疗大面积脑梗死病人的临床研究

目的探讨治疗大面积脑梗死病人的最佳方法。方法大面积脑梗死病人60例。均行开颅去骨瓣减压术；术后分为治疗组（30例）和对照组（30例）。对照组予甘露醇、罗氏芬等常规治疗，治疗组在对照组治疗基础上，于术后9d加用针刺（采用“醒脑开窍”针刺法：取内关、水沟、三阴交、极泉、委中等穴，并合用头针和体针）治疗10d（每日1次），以神经功能缺指评分法及临床疗效评定标准进行治疗前后的功能评价。结果手术病死率为20％；术后采用针刺法能明显改善病人的神经功能缺损程度，治疗组总有效率为40．0％，对照组总有效率为13．3％（P〈0．05）。结论开颅去骨瓣减压术能降低大面积脑梗死病人的病死率。采用手术和针刺配合西药治疗大面积脑梗死有协同作用。

Combination of Operation with Acupuncture for Massive Cerebral Infarction

Objective To approach the best therapy for the massive cerebral infarction.Methods Sixty patients with massive cerebral infarction were treated by surgical decompression. After operation the pts were divided into treatment group(n=30) and control group(n=30) randomly. The pts in control group were treated by routine western medicine (Mannitol, Caftriaxone Sodium etc).The pts in treatment group were treated by the regular therapy plus acupuncture at 9 days. Neiguan Shuigou, Sanyinjiao, Jiquan and Weizhong points were used for 10 days. The neuro-function was assessed by Chinese Stroke Seale Rating method.Results After Surgical decompression,the death rate of the massive cerebral farction was 20%.Total effective rate in the treatment group was 40.0% and that in the control group was 13.3% with a very significant difference between the two groups (P<0.05).Conclusion The Surgical decompression was able to decrease the death rate of the massive cerebral infarction;Acupuncture with western medicine was effective for treatment of massive cerebral infarction.
