枕颈减压、脊髓空洞穿刺抽液治疗Chiari畸形合并脊髓空洞症

目的：探讨一种治疗Chiari畸形合并脊髓空洞症的新手术方法。方法：61例经MRI诊断为Chiari畸形合并脊髓空洞症患，均采用枕颈减压加脊髓空洞空刺抽液术治疗。结果：全部病例术后临床症状，体征均有不同程度改善；随访48例（术后1－5年），明显转好45例，好转3例，其中8例术后2－4年经MRI复查，脊髓空洞消失2例，空洞明显缩小5例，轻微缩小1例，手术无并发症，无死亡，结论：枕颈减压加脊髓空洞空刺抽液术治疗Chiari畸形合并脊髓空洞症是一种有效的方法。

Occipitocervical Decompression Combined with Drainage for Treatment of Chiari Malformation Accompanied by Syringomyelia

Objective To explore a new therapeutic method for treatment of Chiari's malformation complicated by syringomyelia. Method Data of 61 cases, in whom occipitocervical decompression combined with the intracavitary drainage was performed for the treatment of Chiari malformation complicated with syringomyelia, were analyzed retrospectively. Results The clinical signs and symptoms had been differently improved after the operation in all the cases. Follow up (1-5 years) findings in the 48 cases showed that the outcomes were good in 45 patients, and improved in another 3 patients. The postoperative MRI findings in the 8 patients (2~4 years after surgery) demonstrated that the cavities in the spinal cords disappeared in 2 cases, markedly dwindled in 5 and slightly reduced in another cases. There was no complication or death in all the patients. Conclusion Occipitocervical decompression combined with the cavitary drainage is an effective method for treatment of Chiari's malformation accompanied by syringomyelia.
