同步录像脑电图在新生儿惊厥中的应用价值

目的:评价同步录像脑电图(VEEG)在新生儿惊厥中的应用价值。方法:应用VEEG仪,对64例胎龄28~44周的惊厥发作新生儿进行VEEG监测分析。结果:64例惊厥新生儿VEEG监测正常43例(67.2%),异常21例(32.8%)。其中发作期样放电者13例;发作间期样放电3例,其背景节律正常;发作间期背景节律改变伴有样放电2例;仅背景波异常而无样放电3例,其中低电压1例,平坦波1例,交替现象消失1例。睡眠期样放电的放电频率比清醒期明显增高,且以安静睡眠期最为多见。18例VEEG同步监测到发作期样放电,其中16例起始放电为局灶起源,以颞区为主(13/18)。46例临床发作时,VEEG监测均无样放电,临床确诊为非癫样发作。结论:VEEG监测对于新生儿惊厥性疾病的诊断和鉴别诊断具有重要价值,有利于早期治疗和改善预后。

The Value of Video Electroencephalogram in Neonatal Convulsion

Objective:To evaluate value of Video electroencephalogram(VEEGs) in neonatal convulsion.Methods:VEEG monitoring was performed on 64 neonatal with convulsion seizures.Results:Of 64 cases,VEEGs was normal in 43 cases,and abnormal in 21 cases.The abnormality rate was 32.8%. Epileptiform discharge was found in 13 cases on the ictal VEEGs;3 cases on the interictal VEEGs but they had normal background rhythm; 2 cases has epileptiform discharge in the interictal abnormal background rhythm;other cases only had abnormal background rhythm without epileptiform discharge.Discharge frequency in sleeping was more than in wakefulness, which mostly occurred in the quiet sleep phase.Of 18 cases with ictal epileptiform discharges,16 cases had a focal origin(13 cases happen in the temple lobe).Conclusions:VEEGs has important values for the diagnosis and differential diagnosis of epilepsy and non-epileptic seizure in newborn ,and are helpful to early therapy and able to improve prognosis.
