计算法与应用99mTc-DTPA清除率测定肾小球滤过率的临床对比研究

目的 评价临床应用计算法测定肾小球滤过率（GFR）的准确性。方法 用^99mTc-DTPA清除率测定60例不同疾病住院病人GFR（Tc-GFR)，并测血清肌酐，同时以WCP公司测定GFR（WCP-GFR),以Cockcroft/Gault公式计算内生肌酐清除率（CG－CCr)。肾功能不全及正常组WCP－GFR、CG－GFR分别与Tc-GFR进行相关分析，比较两组WCP－GFR、CG－GFR与Tc-GFR间显著性差异以及WCP－GFR、Tc-GFR间平均差和CG－GFR、Tc-GFR间平均差的显著性差异。结果 WCP－GFR及CG－CCr均可在一定程度上准确反映GFR，而以WCP－GFR数值更接近Tc-GFR，准确性更高，临床上可代替Tc-GFR。结论 WCP－GFR是一种准确、简便、快捷测定GFR方法。

The clinical contrast study of determining glomerular filtration rate by calculation and using clearance rate of99mTc-DTPA

Objective To evaluate accuracy of determining glomerular filtration rate(GFR) by using the calculation method clinically.Methods GFRs from 60 patients in hospital suffering from different diseases were determined accurately using clearance rate of 99m Tc DTPA(Tc GFR),and the serum creatinine were determined and simultaneously GFRs calculated by WCP formula(WCP GFR) and creatinine clearance by Cockcroft/Gault formula(CG CCr).The correlation analysis was done between WCP GFR,CG GFR and Tc GFR in the group of renal failure and group of normal.The difference was compared between WCP GFR,CG GFR and Tc GFR and between the average odds of WCP GFR and Tc GFR and those of CG GFR and Tc GFR in two group.Results WCP GFR and CG CCr could reflect GFR accurately to some extent and WCP GFR amount was more proximate to Tc GFR and more accurate than other and could replace Tc GFR clinically.Conclusion WCP GFR is an accurate and simple method to determine GFR.
