帕罗西汀与阿米替林治疗脑卒中后抑郁的临床研究

目的比较帕罗西汀与阿米替林治疗脑卒中后抑郁的疗效及副反应.方法:48例脑卒中后抑郁(PSD)病人被随机分为帕罗西汀组(24例)和阿米替林组(24例),进行为期6周的治疗观察,以汉密尔顿抑郁量表(HAMD)、副反应量表(TESS)于治疗前及治疗后1周、2周、4周、6周末分别对疗效及对副反应进行测评.结果帕罗西汀组显效率为83 .3%,阿米替林组显效率为79.2%,两组显效率比较无统计学意义(P<0.05).两组TESS评分治疗前后组内比较均有统计学意义(P<0.001),两组间HAMD评分比较无统计学意义(P>0.05).帕罗西汀组副反应的发生率及种类明显低于阿米替林组.结论帕罗西汀与阿米替林对脑卒中后抑郁均有良好疗效.帕罗西汀具有副反应小、治疗依从性较好等特点,是治疗PSD的理想药物.

Comparative Study on Paroxetine and Amitriptyline in Treatment of Post-stroke Depression

Objective To study the difference in therapeutic effects and side effects between Paroxetine and Amitriptyline in treating Post-stroke Depression (PSD).Methods To divide 48 cases of PSD patients into two groups randomly, one group therapies by Paroxetine , another by Amitriptyline,then test the patients with Hamilton rating scale for depression (HAMD) and Treatment Emergent Symtom Scale (TESS) before treatment and at the end of the first weekend,the second weekend,the forth weekend and the sixth weekend.Result The effective rate of Paroxetine was 83.3%, the effective rate of Amitriptyline was 79.2%.There was no difference in two groups (P>0.05). Two groups' HAMD accession both had significant difference in pre-and post-treatment(P<0.001),but there was no difference in two groups (P>0.05).The side effects in group Paroxetine were lower than in group Amitriptyline. Conclusion Paroxetine and Amitriptyline have good effects in treating PSD . Paroxetine has fewer side effects compared with Amitriptyline.
