基于影像学技术的脉位属性形成机制研究

摘    要:目的应用现代影像学方法,即时、直观、定量研究脉位属性的形成机制。方法选择年龄在20~30岁之间的健康青年,采用成熟的脉位浮变、沉变无创造模方法,应用NX-8型可视化脉动信息采集分析仪、Magnetom Avanto1.5T超导磁共振成像仪、SONOS-1000型彩色多普勒血流显像仪测定寸口桡动脉血管运动状态、血流动力学及整体循环系统血流动力学相关指标。结果脉位浮变时寸口桡动脉明显扩张,其横截面的直径和面积均明显增大,轴心位移增大,管上组织厚度变薄;桡动脉最大血流速度、平均血流速度减小;心率加快,平均动脉压、脉压减小。脉位沉变时寸口桡动脉横截面的直径和面积均减小,轴心位移减小,管上组织厚度增大,桡动脉最大血流速度、平均血流速度、收缩期、舒张期血流峰值均显著减小,阻力指数明显增大;平均动脉压、总外周阻力升高,脉压、每搏输出量减少,心指数下降。结论脉位的形成与心脏泵血量、血管张力及外周阻力、脉管系统的血液充盈、切脉局部血管的管径及横截面积、轴心位移幅度、管上组织厚度等因素有关。

Abstract:Objective To study the formative mechanism of pulse position property immediately,directly and quantitatively by using the modern image technology.Methods The mature and non-injury method was applied to establish the model of floating pulse and deep pulse in healthy young people aged from 20 to 30 years old.The indexes related to radial artery(cun kou)vasomotion,hemodynamics and hemodynamics of the whole circulation system were determined by NX-8 video-pulse information collective analyzer,Magnetom Avanto 1.5T superconducting magnetic resonance imaging system and SONOS-1000 color Doppler blood flow imager.Results In the floating-pulse condition the radial artery was dilated obviously,the diameter and area of radial artery cross-section were enlarged,the axial displacement was increased,the vessel tissue was changed to thinner.The maximum-portal flow velocity(Vmax)and mean-portal flow velocity(Vmean)of radial artery were decreased,heart rate was increased,and mean arterial pressure and vessel pressure were reduced.In the deep-pulse condition the diameter and area of radial artery cross-section were decreased,the axial displacement was reduced,the vessel tissue was changed to thicker,Vmax,Vmean,peak systolic velocity and peak diastolic velocity of radial artery were all reduced obviously,resistance index(R1)was increased obviously,mean arterial pressure and total peripheral resistance were risen,and the vessel pressure,stroke volume(SV)and heart index were decreased.Conclusion The formation of pulse position is related to some factors,such as cardiac output,vessel tension,peripheral resistance,blood filling of vessel system,diameter and area of radial artery cross-section of radial artery,axial displacement and vessel tissue thickness.
