无创及有创中心动脉压测定的临床对比研究

摘    要:目的探讨无创性、有创性测量所得的中心动脉血压与外周动脉血压(SBP)的一致性,了解中心动脉血压无创性测量的应用价值。方法选择拟作冠状动脉造影的50名患者,据有无基础病(高血压、糖尿病等)将患者分为患病组和对照组,采用无创性中心动脉血压(NCSP)测量仪测量受试者的SBP和NCSP,同时用导管测量受试者升主动脉根部的有创性中心动脉血压(CSP)。结果各组NCSP与CSP间差异均无统计学意义(P〉0.05),两者均呈正相关性(P均〈0.01)。各组SBP与CSP间差异有统计学意义(P〈0.01),两者均呈正相关性(P均〈0.01)。各组CSP与动脉收缩压增强指数(AI)均呈正相关性(P〈0.05)。结论 NCSP与CSP正相关,可以代替CSP。NCSP反映机体血压变化的敏感性强于SBP。中心动脉血压与AI呈正相关。

Abstract:Objective To explore the concordance of non-invasive and invasive blood pressure monitoring in the measurement of central and peripheral arterial pressure(SBP) and to evaluate the clinical value of non-invasive central pressure(NCSP).Methods A total of 50 patients who accepted coronary angiography were enrolled.The patients were assigned to sick group and control group according to the accompaniment of basic diseases(such as hypertension,diabetes).SBP and NCSP were measured by using non-invasive blood pressure measurement unit.The invasive central aortic pressure(CSP) were recorded at the ascending aorta during cardiac catheterization.Results There was no significant difference between NCSP and CSP in both groups(P0.05) meanwhile the two indexes were positive correlated(P0.01).There was a significant difference between SBP and CSP in both groups(P0.01) and the two indexes were high positive correlated(P0.01).A high correlation was found between CSP and argumentation index(AI)(P0.05).Conclusion The correlation of NCSP and CSP indicates that CSP can be replaced by NCSP.NCSP is more sensitive in reflecting the level of arterial pressure than CSP.There is a positive correlation between central aortic pressure and AI.
