血清脂联素水平与冠脉病变严重程度的关系

摘    要:目的探讨血清脂联素水平与冠脉病变严重程度的关系。方法对176例可疑冠状动脉粥样硬化性心脏病(简称冠心病)患者行冠状动脉造影检查并检测其血清脂联素水平,根据冠状动脉造影结果及冠脉病变的Gensinig评分将患者进行分组,比较各组血清脂联素水平的差异;用多元逐步回归分析探讨脂联素与传统冠心病危险因素的关系。结果(1)冠心病患者血清脂联素水平[8.0mg/L(2.4—21.3mg/L)]明显低于正常对照组[11.5mg/L(4.3—29.2mg/L),P〈0.01];(2)脂联素水平随着Gensinig评分的升高而降低(P〈0.05,或P〈0.01);(3)多元逐步回归分析显示,脂联素与高密度脂蛋白胆固醇呈正相关,与冠脉病变Gensini’s评分、甘油三酯和收缩压呈负相关(P〈0.01)。结论冠心病患者血清脂联素水平降低,低脂联素血症是冠脉病变严重程度的标志,脂联素具有心血管保护作用。

Abstract:Objective To investigate the correlation between serum level of adiponectin and severity of coronary artery atherosclerosis. Methods Coronary angiographies were done and serum adiponectin concentrations were measured in 176 cases with suspected coronary heart disease (CHD). Patients were divided into groups according to the coronary angiographies and Gensini's scores of coronary artery atherosclerosis. The serum levels of adiponectin were compared in different groups, and multiple regressions were used,to analyze the correlation factors of adiponectin. Results Serum adiponectin concentration in CHD group [ 8.0 mg/L(2.4 -21.3 mg/L) ] was decreased, compared with that in control group [ 11.5 mg/L(4. 3 -29. 2 mg/L), P 〈0. 01 ]. With the increase of Gensinig scores, serum levels of adiponectin decreased ( P 〈0. 01 ). Serum level of adiponectin was positively correlated with the high-density lipoprotein cholesterol, while negatively correlated with the Gensinig score of coronary artery atherosclerosis, triglyceride and systolic blood pressure ( P 〈0. 01 ). Conclusion Serum adiponectin concentration was decreased in patients with CHD. Low serum levels of adiponectin reflected the severity of coronary artery atherosclerosis. Adiponectin was a protective factor of cardiovascular system.
