临床药师参与顺产临床路径制订与实施效果评价

目的评价临床药师参与顺产临床路径(CP)用药方案制订、路径实施的作用和效果,探索药师参与CP的新模式。方法根据循证医学证据、国内外指南,建立顺产CP用药方案。将纳入顺产CP的患者按随机数字表法分为观察组(100例,临床药师参与临床路径用药方案的制订与实施)和对照组(100例,未予干预),比较两组孕妇的住院费用、药品费用、药占比、住院天数、药品不良反应发生率及患者满意度。结果观察组孕妇的住院费用、药品费用显著低于对照组(P<0.05),住院天数显著短于对照组,药品不良反应发生率及药占比显著低于对照组,患者满意度显著高于对照组(P<0.05)。结论药物治疗作为实施CP管理中的关键环节,有必要根据情况制订精准化用药方案,临床药师参与顺产CP用药方案的制订与实施可明显促进患者安全、有效和经济用药。

Effects of Clinical Pharmacists Participating in the Development and Implementation of Spontaneous Delivery Clinical Pathway

Objective To evaluate the effect of clinical pharmacists participating in the management of clinical pathway(CP)of spontaneous delivery and to explore a new mode of pharmaceutical care for CP.Methods According to evidence-based medical data and clinical guidelines,a standardized and precision medication program for clinical pathway of spontaneous delivery was established.Patients with spontaneous delivery from the hospital were collected and divided into the observation group(100 cases,using medication plan we established this time and with clinical pharmacists participating in the implementation of CP)and the control group(200 cases,no intervention was performed)according to the random number table method.The hospitalization days,hospitalization expenses,drug costs,incidence of adverse drug reaction(ADR),patient satisfaction rate were compared between two groups.Results The hospitalization cost and medicine cost of the pregnant women in the observation group were significantly lower than those in the control group(P<0.05),the hospitalization days were significantly shorter than the control group,the incidence of ADR and the proportion of drugs were lower than the control group,and the patient satisfaction was significantly higher than the control group(P<0.05).Conclusion As a key link in the implementation of CP management,drug therapy is necessary to formulate a precise medication plan according to the situation.The participation of clinical pharmacists in the formulation and implementation of the CP medication plan for natural birth can significantly promote patient safety,effective and economic medication.
